What Is Milprazon for Cats?

Milprazon is a broad-spectrum oral wormer for cats manufactured by KRKA, combining two active ingredients in a single tablet:

  • Milbemycin oxime 16mg — a macrocyclic lactone effective against roundworms, hookworms, and heartworm larvae
  • Praziquantel 40mg — a cestocide effective against all major tapeworm species including the fox tapeworm (Echinococcus multilocularis)

This pack contains 2 tablets (the large cat formulation for cats over 2kg). Milprazon is the generic equivalent of Milbemax (Elanco) — identical active ingredients, same doses, significantly more accessible price.

How the Two Active Ingredients Work

Milbemycin Oxime — Nematode and Heartworm Larva Killer

Milbemycin oxime is a macrocyclic lactone that kills parasitic nematodes (roundworms, hookworms) by binding to glutamate-gated chloride ion channels in invertebrate nerve and muscle cells. This causes permanent hyperpolarisation and paralysis of the worm’s nervous system, leading to death. At the monthly preventive dose, it kills heartworm larvae (microfilariae and L3/L4 stages) before they can develop into adult worms in the heart and pulmonary arteries.

Praziquantel — Tapeworm Killer

Praziquantel works by a completely different mechanism: it disrupts the tegument (outer surface) of tapeworms, dramatically increasing their cell membrane permeability to calcium. This causes rapid spastic paralysis of the tapeworm’s musculature and disintegration of the worm’s body surface. The disintegrated worm is then digested or expelled in the faeces. Praziquantel is effective against all major tapeworm species including Echinococcus multilocularis — the fox tapeworm that causes alveolar echinococcosis, a serious zoonotic disease transmissible to humans. This makes Milprazon particularly important for cats that hunt or spend time outdoors in fox-endemic areas.

Dosage for Cats

⚠️ Give with food — administering with or after a small amount of food significantly improves absorption of both milbemycin oxime and praziquantel, and also ensures optimum heartworm prevention efficacy.

This product is for cats 2kg and over. For kittens under 2kg or under 6 weeks of age, use a kitten-appropriate worming preparation.

How Often to Give Milprazon

  • Routine worming: Every 1–3 months depending on lifestyle and risk. Outdoor cats that hunt and scavenge may need monthly treatment.
  • Heartworm prevention: Once monthly throughout the mosquito season (or year-round in endemic regions). Begin one month before first mosquito exposure and continue for at least one month after the last.
  • Echinococcus control: At least every 4–6 weeks in cats at risk of exposure to rodents or fox faeces.

⚠️ Important Warnings

Heartworm Test Required Before Starting

If your cat lives in a heartworm-endemic region and has not previously received preventive treatment, they should be assessed for existing heartworm infection before starting Milprazon. Administering milbemycin oxime to a cat with a high burden of circulating heartworm microfilariae can cause a shock-like hypersensitivity reaction (due to the rapid death of large numbers of microfilariae releasing their proteins) — characterised by vomiting, salivation, pale mucous membranes, laboured breathing, and weakness. Consult your veterinarian in heartworm-endemic areas before first use.

Does Not Kill Adult Heartworms

Milprazon prevents heartworm infection but does NOT eliminate adult heartworms already established in the heart and pulmonary vessels. If adult infection is present, separate adulticidal treatment is required.

Minimum Age and Weight

Do not use in kittens under 6 weeks of age or under 0.5kg body weight.

Side Effects

Milprazon is well tolerated in cats at the recommended dose. Rare reactions include:

  • Transient vomiting or diarrhoea after administration
  • Hypersalivation (if the tablet is partly chewed rather than swallowed)
  • Neurological signs (very rare; more likely if combined with P-glycoprotein inhibitors)

Drug Interactions to Be Aware Of

The following drugs can inhibit P-glycoprotein and potentially increase milbemycin oxime levels to toxic concentrations, particularly in genetically sensitive individuals:

  • Cyclosporine (Cyclavance, Atopica)
  • Azole antifungals (ketoconazole, itraconazole)
  • Diltiazem, amiodarone, erythromycin

Inform your veterinarian if your cat is receiving any of these medications before starting Milprazon.

Storage

  • Store at room temperature, below 25°C
  • Keep in the original blister packaging and away from moisture and direct sunlight
  • Keep out of reach of children and other animals
